Anatomy of the Middle Ear
Tympanic Cavity (Middle Ear)
The tympanic cavity is a space located in the petrous part of the temporal bone. Its walls are as follows:
- External Wall: Formed by the tympanic membrane (eardrum). The space superior to the membrane is the epitympanic recess (attic).
- Internal (Medial) Wall: This wall separates the middle ear from the inner ear. It features the promontory (a central bulge) and two openings: the oval window (fenestra vestibuli) and the round window (fenestra cochleae). These windows communicate with the inner ear cavities and are sealed by the stapes (oval window) and the secondary tympanic membrane (round window).
- Anterior Wall: Contains the opening for the Eustachian tube (auditory tube), which connects to the nasopharynx. It has a bony portion and a fibrocartilaginous portion and serves to equalize pressure with the external auditory canal.
- Posterior Wall: Features an opening (aditus ad antrum) that leads to the mastoid antrum, from which the mastoid air cells radiate.
Chain of Auditory Ossicles
The middle ear contains three small bones (ossicles): the malleus (hammer), incus (anvil), and stapes (stirrup). They form a chain that transmits mechanical vibrations from the tympanic membrane to the oval window. This mechanism is regulated by two muscles:
- Tensor Tympani Muscle: When it contracts, it increases the tension of the tympanic membrane and the pressure exerted by the stapes.
- Stapedius Muscle: Its contraction reduces the tension of the tympanic membrane and the pressure exerted by the stapes.
Anatomy of the Urinary System
Ureter
The ureter descends on the psoas major muscle and crosses the abdominogenital nerve. It then passes over the external iliac vessels to enter the pelvis. In its pelvic course, it crosses the uterine vessels in females or the vas deferens in males before entering the posterior wall of the bladder, opening at the ureteral orifice.
